Cannot submit form using Firefox 54 on Fedora 25. Submit button does not activate.

Did you mean the 'Continue' button on the first page? If you select a quantity no less than 5 and enter the company name the 'Continue' button should be enabled. The quantity is just the number of users. If you have less than 5 users you don't need to buy the Fair Source license.
